Care Management

Professional care management is a holistic, client centered approach to caring for older adults. Our goal is to enhance the quality of life and reduce stress for our clients. Our Care Manager is a member of the Aging Life Care Association.

Services may include:

Coordination of health care needs
Coordination of health care appointments
Accompany to doctor appointments
Health insurance questions
Management of transitions from hospital to home
Assist with adding additional home health services
Make referrals for active equipment

Organization of services designed to promote health and independence
Visits to monitor needs/outcomes
Facilitator for all interested parties
Advocacy
Bill paying and organizational support
Emergency planning
Community resources

What is a Care Manager?

Our Care Manager partners with seniors and their families to help manage the many challenges and transitions associated with aging and healthcare. Our Care Manager understands every situation is unique and respects the autonomy and the dignity of each individual.

Our St. Camillus Care Manager has many years of experience and expertise to improve the quality of life for those we serve.

Our partnership may include planning, arranging, and coordinating support for the client. We carefully monitor the type and value of the services to provide the most effective outcomes.
