Professional care management is a holistic, client centered approach to caring for older adults. Our goal is to enhance the quality of life and reduce stress for our clients. Our Care Manager is a member of the Aging Life Care Association.

  • Coordination of health care needs
  • Coordination of health care appointments
  • Accompany to doctor appointments
  • Health insurance questions
  • Management of transitions from hospital to home
  • Assist with adding additional home health services
  • Make referrals for active equipment

What is a Care Manager?

Our Care Manager partners with seniors and their families to help manage the many challenges and transitions associated with aging and healthcare.  Our Care Manager understands every situation is unique and respects the autonomy and the dignity of each individual.

Our St. Camillus Care Manager has many years of experience and expertise to improve the quality of life for those we serve.

Our partnership may include planning, arranging, and coordinating support for the client.  We carefully monitor the type and value of the services to provide the most effective outcomes.
